Care at home or in the community can begin after hospital, rehabilitation center or skilled nursing home stay, or after post-op care or an illness, physical or mental disability that limits a person's functional abilities in the area of eating, bathing, dressing, transferring, etc. Care is provided in the comfort, privacy and safety of your home, congregate setting or in an assisted living facility.
